As per OHIP guidelines, a new referral from a physician / NP is required after 2 years from the initial visit to allow continuation of ongoing care.

Please include patient demographics, OHIP #, reason for referral, relevant history, and the patient's email address and cell phone number (both mandatory).
Refer through Ocean
Send a secure referral to Dr. S. R. Abd Halim's Psychiatric Clinic (Virtual Only) directly from your EMR or the Ocean Healthmap listing.
For virtual care extending beyond 24 months, OHIP and the CPSO require that a physician/NP issue a new re-referral for continued virtual care.
We are unable to provide psychiatric consultation for court, custody, or insurance purposes, or a second opinion not being requested by the current treating psychiatrist.
