Clinician-administered · Version 8.1

Montreal Cognitive Assessment (MoCA)

A 30-point screen of cognition administered and scored by the clinician during the appointment. The patient draws the trail making, cube copy and clock tasks directly on screen.

For clinician use only. The version 8.1 wording (word list, digits, letter list, sentences and similarity pairs) is shown below and must be read aloud by the rater; the naming pictures, the trail-making layout and the cube sample are shown on screen. The MoCA is copyright MoCA Cognition and is used under the clinic's licence.

Patient & session details

Visuospatial / Executive

Alternating trail making, cube copy and clock drawing (ten past eleven). The patient draws directly on screen with a mouse, stylus or finger.

Alternating trail making

"Please draw a line, going from a number to a letter in ascending order. Begin here (point to 1) and draw a line from 1 then to A then to 2 and so on. End here (point to E)."

Nothing drawn yet.
Copy cube sample

Copy this drawing

Copy cube

"Copy this drawing as accurately as you can, in the space below." (Show the version 8.1 cube beside the drawing area.)

Nothing drawn yet.
Draw CLOCK (ten past eleven)

"Draw a clock. Put in all the numbers and set the time to 10 past 11." (3 points: contour, numbers, hands.)

Nothing drawn yet.
Alternating trail making (1-A-2-B-3-C-4-D-5-E)

1 point only if the entire sequence is drawn without any error and without any line that is not immediately corrected.

/ 1
Copy cube

3-dimensional, all lines drawn, no line added, lines relatively parallel and of similar length.

/ 1
Clock — contour

The face must be a circle with only slight distortion.

/ 1
Clock — numbers

All 12 numbers present, no extras, correct order and placed in the approximate quadrants.

/ 1
Clock — hands (ten past eleven)

Two hands jointly indicating the correct time; the hour hand clearly shorter than the minute hand, joined near the centre.

Naming

Point to each picture below and ask: "Tell me the name of this animal." The official version 8.1 drawings are shown on screen in order.

Show accepted answers (rater only)

1. Lion · 2. Rhinoceros (rhino) · 3. Camel (dromedary)

Memory — registration (no points)

Read the list FACE, VELVET, CHURCH, DAISY, RED at one word per second; the patient repeats them. Do 2 trials even if the first trial is successful. Tell the patient you will ask for the words again later. No points are given here.

1st trial — tick each word the patient repeats
2nd trial — tick each word the patient repeats

Attention

Digit span, vigilance (tap at each letter A) and serial 7 subtraction from 100. Read at one digit or letter per second.

Digits forward — 2 1 8 5 4
/ 1
Digits backward — 7 4 2
/ 1
Vigilance — tap at each letter A: F B A C M N A A J K L B A F A K D E A A A J A M O F A A B

No points if there are 2 or more errors (a tap on a wrong letter or a missed A).

/ 1
Serial 7 subtraction starting at 100 (93, 86, 79, 72, 65)

4 or 5 correct subtractions = 3 pts; 2 or 3 correct = 2 pts; 1 correct = 1 pt; 0 correct = 0. Each subtraction is judged from the previous number given.

/ 3

Language

Sentence repetition and phonemic (letter F) verbal fluency in one minute.

Repeat: "I only know that John is the one to help today."
/ 1
Repeat: "The cat always hid under the couch when dogs were in the room."
/ 1
Fluency — maximum number of words in one minute beginning with the letter F

1 point for 11 or more words in 60 seconds. Exclude proper nouns, numbers and repetitions with a different suffix.

/ 1

Abstraction

"Tell me how they are alike." Practice item: banana – orange = fruit. Only the last two pairs are scored.

train – bicycle

Accept: means of transportation, means of travelling, you take trips in both.

/ 1
watch – ruler

Accept: measuring instruments, used to measure.

Delayed recall & Memory Index Score

"I read some words to you earlier, which I asked you to remember. Tell me as many of those words as you can remember." Score 1 point for each word recalled with no cue. Category and multiple-choice cues may then be given: these score no MoCA points but feed the Memory Index Score (MIS, out of 15).

Orientation

"Tell me today's date." Then ask for any part not given: month, year, day of the week, place and city.

Upload the patient's completed tasks

Attach the single PDF produced by the patient's Cognitive Drawing & Naming Tasks page (trail making, cube copy, clock drawing and the three naming answers). One file, up to 10 MB. A new upload replaces the previous one.

Read the uploaded sheet

The trail making, cube, clock and naming items — and the patient's details — are filled in automatically as soon as a file is attached. Use the button to read it again. This is a reading aid only: every score must be reviewed and confirmed by the clinician, whose judgement is the score of record.

MoCA total 0/30. Severely below cut-off — a range often seen in severe cognitive impairment. The MoCA is a screening instrument and is not diagnostic on its own; interpret alongside history, collateral information and functional assessment.
